How to Refer

2. Provide Details: Share participant information and support needs
3. Assessment: We will conduct an assessment to understand requirements
4. Service Agreement: We will prepare a service agreement and begin support

• NDIS plan details and funding categories
• Support coordinator or LAC contact
• Current support needs and goals
• Any relevant medical or behavioral information
